A structured, one-time consultation for families navigating complex adult care or hospital discharge decisions.

This service includes:

• Review of relevant documentation (discharge summaries, therapy recommendations, insurance information, guardianship documents, etc.)

• A 75-minute consultation session

• Clarification of level-of-care distinctions (acute rehab, SNF, home health, etc.)

• Identification of key decision points and risk considerations

• A written Action Map delivered within 48 hours outlining priorities, sequencing, and questions to bring back to providers
  
  

This service does not include:

• Medical or legal advice

• Representation

• Facility placement recommendations

• Direct communication with hospitals, facilities, or insurers

Pricing

Pilot Rate:

$150 per consultation

(75-minute session + written Action Map)

Payment is required at the time of booking.

Cancellations with more than 24 hours notice may reschedule once. Sessions canceled within 24 hours are non-refundable.
